Within 80 yards of the green is your “scoring zone” and you should be focused on getting down in no more than two shots. Firstly, when aiming make sure your stance is slightly open. This helps you swing on a slight out-to-in path which creates a higher ball trajectory and greater control. Your shoulders remain parallel to your ball to target line still. In your stance turn your right toe in a fraction, this prevents too much leg action occurring in the shot and therefore giving a solid base to give consistently clean and accurate strikes. With the ball slightly back of middle in the stance place a little more weight on your left side to ensure that your sternum is directly over the middle of your stance at address. Due to the change in set up, the backswing will be slightly shorter. Try to create a smooth but accelerating swing making sure you swing through to a full finish position. Judging the distances of your pitching can be helped by carrying a few different lofts of wedges. Most professionals these days will carry at least 3 or 4 wedges in their bag for greater shot options. If you have a good spread of wedges ie, from a 46 degree pw to a 60 degree lob wedge you are likely to hit the right yardages far more consistently. Practice from various distances out from the green and just try to "feel" how hard you need to hit each shot by pretending to throw a ball with your right hand while looking at the target to find the required yardage, remember practice will make perfect.
